add merge request template

We tried to add the file in .gitlab/merge_request_templates/Default.md as stated in Gitlab documentation, the template appears in the template dropdown but isn't selected by default.

In the project settings the option to do that is not present, @famarger, @arthur, should we ask Octobus to have a look?

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
